New and current patients can call the Mercy Health COVID-19 Vaccination Hotline at 866-624-0366 on Monday through Friday from 8 a.m. to 5 p.m. to schedule their appointment. Mercy Health patients can also schedule their appointment through MyChart.
Attendees will need to bring these to their appointment:
- A mask (if you do not have a mask, one will be provided)
- Form of identification
- Insurance card
- An attestation form is available for you to print and complete prior to your appointment.
Vaccinations are available by appointment only to patients who meet eligibility criteria. Vaccines are limited and call volumes are high. The number of appointments can vary based on the amount of vaccine Mercy Health receives.
